We Can Help You Find Those Tax Forms @ the Library
Main Library: Appointments with the tax preparers will start on Wednesday, February 1, 2012 and run through Thursday, April 12, 2012. You can call to schedule your appointment at 608-789-7443 from 9-3 p.m. Tuesday-Thursday until April 12.

South Community Library: The AARP tax group will be providing assistance with Homestead tax forms on these dates and at these times:
Monday, February 27, taking appointments from 1-4 p.m. and Monday, March 5, taking appointments from 1-4 p.m. Reservations can be made by calling 789-7103 during normal branch open hours: Monday through Wednesday noon to 8 p.m. and Thursday through Saturday 10-5. You can start calling for an appointment - 789-7103.
AARP-Tax-Aide volunteers prepare federal and state returns for middle and low-income taxpayers, with special attention given to people 60 and older. This Tax-Aide site is a joint venture with RSVP and LPL volunteers staffing the appoinment phones, AARP tax preparers, and the La Crosse Public Library.

Requesting a Tax Form to be Sent to You
Federal
You can order many of the federal tax forms and publications by visiting this page on the IRS's website. Or you may request tax forms by calling 1-800-829-3676 or write to the following address: Internal Revenue Service, 1201 N. Mitsubishi Motorway, Bloomington, IL 61705-6613. You should receive a response within 10 days after your request is received.
Wisconsin
You can order many of the Wisconsin tax forms and publications through their online service. You can also request forms by writing to: Wisconsin Department of Revenue, 2135 Rimrock Road, Mail Stop 5-77, PO Box 8949, Madison Wi 543708-8949. You can also call 608-266-1961.
